Part Two: Complete and Return the Parent/Guardian Consent Form

• Go "old school," download, print, complete and snail mail the 2017 waiver form to the diocesan office: Episcopal Diocese of Nebraska, 109 N 18th Street, Omaha, Ne 68102.
• Go "Paperless" download and complete the PDFs, then scan and email it to nptomey@yahoo.com.

Part Three: Complete Payment

The cost of the GROW Event is $20 which  includes supplies, lunch and snack for the day.

At the conclusion of the Wufoo form, you will have the option to pay via PayPal - follow the guided steps to complete online payment. You will need a PayPal account to pay online.

Send a check, made out to the Episcopal Diocese of Nebraska,  (GROW in the memo line), to the Episcopal Diocese of Nebraksa, 109 N 18th Street, Omaha, Ne 68102.
